If you are a new patient we have a confidential patient form that we would like you to fill out and bring with you. This form is availabe in PDF format and can be downloaded by clicking here. Please print it out and fill it in prior to your visit. Or, you may FAX it to our office at 904-276-7559 prior to your visit.
If you are a referring physician you can contact us at 904-276-9222 or by email at any time. If you email us kindly include you practice or physician name in the subject area of the email.
Our offices are closed from 12N until 1:30pm ET for lunch each day. You can call 888-500-VEIN if no one picks up at 904-276-9222.
If you choose to email us, please allow one business day for us to get back to you as it is difficult to respond to all of the emails in the course of the day working with patients in the office.
On select days each month we offer varicose vein screenings. If you have never been through a screening for your condition you may find this screening very useful in deciding how to treat your varicose vein discomfort.
